Supervision Disclosure

Licensed Master Social Workers (LMSWs) and Licensed Associate Counselors (LACs) in Arizona are provisionally licensed and provide services under the supervision of a fully licensed LCSW, LPC, or other qualified mental health professional. This supervision ensures that all clients receive care that meets professional and ethical standards while the LMSW or LAC works toward full independent licensure. Clients may occasionally receive feedback or consultation from the supervising clinician, but all treatment is provided directly by the LMSW or LAC. This process is standard for provisional licensees and is designed to support high-quality, safe, and effective care.

Session Details & Payment

  • Sessions are typically 50–60 minutes
  • Payment is due at the time of service
  • Accepted payment methods may include credit/debit card and HSA/FSA (where applicable)
  • Fees are subject to change with advance notice

Insurance & Reimbursement Information

Some insurance plans may provide partial or full reimbursement for out-of-network mental health services. Our practice provides superbills to clients who pay privately, which can be submitted to your insurance company for potential reimbursement. Please contact your insurance carrier directly to determine coverage, eligibility, and any out-of-network benefits.

No Surprises Act & Good Faith Estimates

In compliance with the No Surprises Act, our practice provides patients with a Good Faith Estimate (GFE) of expected charges for mental health services. This estimate is provided before services are rendered to help you understand your potential financial responsibility.

The Good Faith Estimate includes:

  • The cost of the service(s) you are scheduled to receive
  • Any applicable fees for private pay (self-pay) sessions
  • A clear statement of whether the provider is in-network with your insurance

If you do not have insurance or choose to pay out-of-pocket, this estimate gives you the ability to compare costs across providers and make informed financial decisions.

Important Notes

  • The estimate is based on the information available at the time and may change if your treatment plan changes or additional services are required.
  • You have the right to receive a Good Faith Estimate for any scheduled service.
  • If your bill differs significantly from the estimate, federal law provides options for dispute resolution.
